We believe healthcare should be accessible, affordable, and rooted in compassion. Whether you’re navigating financial uncertainty or need help accessing essential services, Chambers Health offers a variety of assistance programs to help meet your needs.
From sliding fee discounts to community-based support, we’re here to remove barriers so that you can focus on your health and well-being.


Sliding Fee Program
At Chambers Health, we are committed to making healthcare accessible for everyone. As a Federally Qualified Health Center (FQHC), we provide services to all patients, regardless of income, insurance status, or ability to pay.
Our Sliding Fee Program helps reduce the cost of care for individuals and families who qualify. Discounts are based on household size and income, in accordance with Federal Poverty Level guidelines.
Uninsured patients will meet with a Benefits Specialist during their first visit to:
- Complete a brief income screening
- Explore eligibility for available assistance programs
- Determine the appropriate fee based on our sliding scale

Grant Applications
Chambers Health may offer limited grant-funded assistance to cover specific services for qualifying patients. Our team can guide you through the application process and determine if you’re eligible.

Chambers Cares Program

Program Overview
Services include:
- Home-delivered meals brought directly to your door
- Congregate meal gatherings for adults 60+ to enjoy food and friendship
- Emotional and social support through our team of trained Community Health Workers
- Resource navigation to help you access local services and aid programs
Home-Delivered Meals: Who Qualifies?
To receive home-delivered meals, individuals must:
- Be age 60 or older
- Complete a Consumer Needs Evaluation form
- Score 20 or higher based on the ability to complete daily living tasks independently
This evaluation helps determine if meal delivery is the right fit for your level of need.

Volunteer With Chambers Cares
We rely on the dedication of volunteers to bring our mission to life. Volunteers help with:
- Delivering meals to homebound seniors
- Assisting with congregate meal service
- Offering companionship and wellness check-ins

What Are Social Determinants of Health?
Health is shaped by more than just doctor visits. Social Determinants of Health (SDOH) include factors like:
- Housing and transportation
- Nutrition and food access

The Chambers Cares program works to identify and address these non-medical barriers, ensuring that seniors and at-risk individuals have what they need to stay safe, nourished, and well-connected in their community.
